Wednesday, August 4 from 10:00 a.m. to 8:00 p.m.
Thursday, August 5 from 10:00 a.m. to 8:00 p.m.
Friday, August 6 from 7:30 a.m. to 5:00 p.m.

STUDENT REGISTRATION REQUIREMENTS
2010-2011

Please BRING the following documents with you to registration:

1. Completed Family Residency Form (7:60-E-1) (Available online)
2. Two Current Proofs of Residency from the Columns listed on the 7:60-E-1 Form
3. If a student is new to the District or is returning after having left our District - Original Birth Certificate or Certified Copy of Birth Certificate or Visa if appropriate (will make copies for school records)
4. Payment for Fees (may pay by cash, check or credit card)
5. Student Good Standing Form, (if transferring in from another school district)
6. Physical Exam Form  (Available online)
   If entering Kindergarten, 6th grade, 9th grade, or upon initial enrollment of the student in the Harlem School District. (Proof of physical exam needs to be submitted prior to October 15 or the child will be excluded from attendance).
7. Dental Examination Form (Available online)
   All children in Kindergarten, 2nd grade, and 6th grades must present proof of having been examined by a licensed dentist before May 15 of the current school year in accordance with rules adopted by the Illinois Department of Public Health.
8. Eye Examination Form (Available online)
   All students entering Kindergarten or an Illinois school for the first time shall present proof before October 15 of the current school year that the student received an eye examination within one year prior to entry of kindergarten or an Illinois school for the first time.  A physician licensed to practice medicine in all of its branches or a licensed optometrist must perform the required eye examination.
